WHAT YOU’LL NEED TO SUBMIT The following pages list all required documentation for your credentialing package. If you’ve already submitted any items, that’s great, just double check with us to make sure they meet the facility’s documentation standards. To ensure we hit your facility’s deadlines and avoid delays, it’s essential to maintain regular contact with us throughout the process. Please ensure all documents are submitted by the due date. LICENSURE & CERTIFICATIONS: Professional License for the state of assignment (Compliance Specialist will note if a hard copy is required) BLS Certification MISCELLANEOUS/HR DOCUMENTS: Government Issued Photo ID (Driver License, State ID, or Passport) Social Security Card Education Verification: Required for all licensed positions. This is done through a third party. To expedite the process please send a copy of your diploma or transcripts.

MEDICAL DOCUMENTS: Physical(within 1 year) PPD(Documentation of PPD Skin Tests within 1 year of start date OR 1 QuantiFERON / Tspot blood test within 1 year of start date. If history of positive PPD, you MUST provide proof of past positive, Chest X-ray within 5 years, and an Annual TB Symptom Survey) Hepatitis B(Full series, Positive AB Titer, or Signed Declination Form) MMR(2 vaccines, Positive titer or Signed Declination Form) Varicella(2 vaccines, Positive Titer, or Signed Declination Form) Tdap(Vaccine within 10 years or Signed Declination Form) FLU Vaccine(Seasonal vaccine required during season Oct - March) Drug Screen: You will be provided with the name and location of the lab that will complete your drug screen. Please do not schedule your drug screen until you have been provided with this information

TIMESHEETS You need to submit your hours through Kronos and share with us a screenshot of Kronos every week by Monday. If you face any challenge, feel free to reach out. PAY STUBS You can register and begin viewing your pay stubs on ADP once your first payroll has been processed. We ask that you do not make any changes to your details on ADP, such as updating your bank information, address, or any other personal data as it can inadvertently impact your paycheck. If you need to update any of your personal information, reach out to us directly. We will ensure that your records are updated correctly and securely to avoid any issues with your paycheck. PAYROLL SCHEDULE You have a weekly payroll schedule. Please refer to the attached payroll schedule for pay date and pay period. COMMUTER BENEFIT PROGRAM GUIDE We offer a commuter benefits program through Health Equity. If you wish to enrollor need more information, please view the attached brochure.

401K PLAN You are eligible to participate in our 401k retirement savings plan however we do not currently offer matching contributions. Please find attached 401k enrollment form to complete. Review, fill, and submit directly to your ADP account. Credentials for ADP will be shared once your 1st paycheck is processed.You can review the attached brochure or call ADP 401K Customer Care: 1-800-695-7526. Or complete it online atwww.mykplan.com.

INSURANCE DETAILS Our insurance providers are UnitedHealthcare (medical and prescription), Guardian (dental), and VSP (vision). We offer two medical and prescription plans: UnitedHealthcare MOD Plan UnitedHealthcare Nexus ACO R HSA Plan The plan summaries with the monthly rate charts are attached for your reference. If you enroll, you are responsible for the cost of your selected plan. T4H contributes toward the lowest-cost self-only plan that meets minimum value requirements, ensuring your contribution does not exceed 8.39% of monthly income. Coverage begins on the first of the following month. If you do not enroll now, you may enroll during open enrollment in June every year. You have 30 days from your start date to make your decision. We also offer voluntary benefits through Transamerica, including short-term disability, critical illness, accident, whole life, and term life coverage. For enrollment assistance, contact Farmington Company at 1-844-428-6675 (Mon–Fri, 8am–6pm EST). Please see the attached brochure for details.

LEAVE REQUEST AND APPROVAL Any leave of absence or late arrival must be communicated to both your onsite supervisor and the Talent4Health Employee Relations department by email. Once your supervisor has approved the request, please forward the approval email to the Talent4Health Employee Relations department for our records. CONFLICT RESOLUTION & EMPLOYEE RELATIONS In the event of a workplace conflict with a co-worker or onsite supervisor, employees should contact the Talent4Health Employee Relations department as their primary point of contact and avoid direct confrontation. We will coordinate with the client to address the situation and seek a constructive resolution that protects all parties involved. DRESS CODE On your first day of work, please wear scrubs or dress appropriately for your specific role. Afterward, you may follow the dress standards observed within your team and work environment. We ask all consultants to maintain a professional and respectful appearance while representing Talent4Health at client locations.

T4H LEARNING PORTAL ONLINE TESTING AND SKILLS CHECKLIST You will be assigned exams through our learning portal healthcare staffing hire. Check your email from HSH support@healthcarestaffinghire.com with the subject line “Welcome to the Talent4Health Family!”Please register using the link and complete the required courses at your earliest convenience. T4H HR PAPERWORK AND CLIENT SPECIFIC FORMS Forms sent via Sertifi and completed in T4H’s applicant tracking system, Labor Edge. Check for an email with the subject line “Electronic Signature Request from Talent4Health LLC.” BACKGROUND CHECK A link has been sent to you from our vendor. Once it’s completed, your background check will be in process.
